JL59 PAGCOR Exclusion: Who Can Apply and What to Send

- Choose self or family exclusion
- Step 2
- Choose the period
- Step 3
- Gather the documents
- Step 4
- Fill in PAGCOR's form
- Step 5
- Submit it to PAGCOR
PAGCOR exclusion is a programme the Philippine regulator runs itself, and you apply to PAGCOR, not to a casino. This page sets out the steps from the saved pages of PAGCOR’s regulatory site: what each programme is, who may apply and what the paperwork asks for. If you are still deciding between a pause and an order, start with the bet responsibly checklist.
PAGCOR exclusion in five steps
- Choose between self exclusion and family exclusion.
- Choose the period you want.
- Gather the photo ID, the 2x2 photos and any extra document.
- Fill in the matching PAGCOR application form.
- Send it to PAGCOR by the route its own page lists.
Everything below expands one of these steps, with PAGCOR named as the source of every rule.
Self exclusion: periods and requirements
PAGCOR’s exclusion page describes self exclusion as for patrons who feel they are developing a problem with gambling. You pick the period, and after it ends the order lifts automatically.
| Detail | What PAGCOR’s page says |
|---|---|
| Periods | 6 months, 1 year or 5 years |
| First six months | Irrevocable |
| Form | Self Exclusion application form |
| ID | Photocopy of a government-issued photo ID |
| Photos | Two 2x2 colour photos, taken within the last 6 months, full face, no hat, glasses or facemask |
| Non-personal submission | A selfie or video call is required for confirmation |
Pick the period you can say yes to on your worst day, not your best one.
The PAGCOR self exclusion form and where it goes
PAGCOR’s Responsible Gaming Code of Practice, Version 6.0, names the self-exclusion application as RG Form 2 and says the form can be downloaded through PAGCOR’s regulatory site. If you want to change the period after the first six months, the Code names a Petition to Amend the Exclusion Period, RG Form 8. When the period ends, the Code says you can re-apply on RG Form 2 by ticking the re-application box.
For the route, PAGCOR’s responsible gaming page lists an Online Self-Exclusion Application Portal at osea.pagcor.ph. Its exclusion page also lists an email route and a drop box at the MET Live Building, Central Business Park 1-A, Macapagal Blvd cor. EDSA Extension, Pasay City. Take the form and the current address from PAGCOR’s own site on the day you apply.
Family exclusion: who applies and what to attach
Family exclusion is for a spouse, a child or a parent who needs help with problem gambling. PAGCOR’s Code of Practice calls the family member the applicant and the person with the gambling problem the respondent, and allows an adult child to apply.
- The Family Exclusion application form, downloaded from PAGCOR.
- A photocopy of the respondent’s government-issued photo ID.
- A photocopy of the applicant’s government-issued photo ID or school ID.
- Two 2x2 colour photos of the respondent, taken within the last 6 months.
- A selfie or video call for confirmation, if the submission is not personal.
- The respondent’s birth certificate if the applicant is the parent or child, or a marriage certificate if the applicant is the spouse.
The periods are 6 months, 1 year or 3 years. PAGCOR’s responsible gaming page lists the email address for completed family applications as RGD_exclusion@pagcor.ph.
What the order covers and what happens after
PAGCOR’s exclusion page says exclusion is implemented across all sites licensed and operated by PAGCOR. Its Code of Practice adds that names of excluded persons are posted in the National Database of Restricted Persons, and that the order is enforced in all PAGCOR operated and regulated gaming facilities wherever the application was made. Before you rely on an order for a particular site, look the operator up in PAGCOR’s own published lists.
PAGCOR describes the order in terms of the sites it licenses and operates, so keep a phone timer and a cool off break from betting running as well.
